Eligibility
Applicants should meet the following criteria:

Live in the following areas of northern Rhode Island: Burrillville, Harrisville, Pascoag, Glendale, Mapleville, North Smithfield, Forestdale, Slatersville, Lincoln, Glocester, Chepachet, Harmoney, Smithfield,
North Providence,
Greenville, Esmond, Foster, Scituate, and Johnston.

Be able to live safely in the community, with additional supports.
Enrollment Process

Call and speak with one of our nurses/representatives to discuss your personal situation.

Arrange for a visit our centre or for us to come out on a home visit.

You will be given all the information that you need to begin and we will proceed with the steps to registration.

Plan to make an appointment and set aside at least an hour to fill out all of the necessary paperwork.

Methods of payment will be discussed.

Our representative will work with you to verify illegibility for state assistance.
